This Job Aid for ALR Users shows how to create an Incident Report.
View ALR User Dashboard
Login to Quickbase, and enter the ALR Incident Reporting application. Click the [Home] button to view the ALR User Dashboard.
A reportable incident is either Resident-specific or Facility-wide.
Click either [Add New Resident-specific IR] or [Add New Facility-wide IR].
The Add Individual IR screen displays.
There are five (5) sections to an Incident Report, each Section matching a heading: Status, ALR, Nature of the Incident, Supplemental Documentation, and Submission Settings.
Each heading can be toggled open/close by clicking the small triangle.
The Headings are slightly different between the paper form and the QuickBase Add screen, but the data elements exactly match the fields on the current paper report form (e.g. Form version 1.2, or the latest version published.)
On the Add screen, Status shows the submission status of this IR.
The IR is not visible to the ALR Certification Unit until it is Submitted.
Assisted Living Residence (ALR) section
Fill in all fields.
Nature of Resident-specific Incident
All fields are required.
The Incident Headline should be a short description of the incident.
Depending on the Type of Incident selected, the Specific Type, Detail Type, and Resident Status dropdown fields will contain different options.
Most often, Supplemental Documentation is not available at the time an Incident Report is submitted.
If you wish to include relevant documentation at submission-time, you can attach a file from your computer. For each attached file, you must provide an Attachment Title.
Omit these fields if no documentation is available at submission-time: this infomration cannot be added to the QuickBase record after submission by ALR Users.
These fields are optional.
Check this box if you are ready to submit the Incident Report.
When you click [Save], the system will check for all required fields. If the Incident Report is submitted, it is no longer editable by ALR staff.
If you did *not* check the To Be Submitted checkbox, the Incident Report can be edited or updated later, by you or any other ALR users.
